HOW DO I....
OBTAIN A REPEAT PRESCRIPTION ?

If you require a further supply of medication please return the tear-off computer slip from your previous prescription, indicating which items you require. This may be posted to us or dropped into reception. Please allow two working days before collection or, if you prefer, please enclose a stamped, addressed envelope if you would like it posted back to you. To avoid inaccuracies we do not accept telephone prescription requests. The doctor also likes to review your treatment regularly and the computer slip will tell you when you should make an appointment.

Sickness Certificates

You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may, however, require you to complete a self certification form (SC2), which they will supply. If they require a signed doctor’s certificate for this period, a private one is issued for a fee payable by the employer. For any illness lasting longer than seven days, you will need to see the doctor for a sickness certificate (MED3) to be issued and for any subsequent renewal of the certificate.
